What are the differences between the day to day and responsibilities of working at a smaller healthcare consulting firm (that specializes in healthcare) versus working on a healthcare specific team at a big consulting firm (like mckinsey, BCG, etc).

Is either one more prestigious than the other?

This answer depends on personal preferences - let me list the advantages:

1. Smaller specialised firm:
+ leaner (smaller) team means you'll learn significantly more and gain more responsibility (deal with the client)
+ you can potentially rise through the ranks and get promoted quicker
+ pay tends to be higher because you're on a smaller team
+ you'll learn more healthcare specific terms and more about the space and up-and-coming players

2. Bigger firm
+ prestige and excellent exit opportunities
+ great future network
+ ability to work with bigger, well-known clients
+ you can start as a generalist and switch to trial other industries if you're not 100% sure on healthcare

In general one may be prestigious over an other, however it's hard to tell unless you analyse client flow (definitely try and read the news and ask mentors in the space about their opinions).
